Its not sailing, but Roz Savage's attempt to row across the Pacific certainly qualifies as a small boat, long distance voyage!  She's doing cool things with web 2.0 type media, including daily blogging, podcasts 3x a week, and even short video clips.  She's now several weeks out of San Francisco on her way to Hawaii.  Her web site is http://www.rozsavage.com/
