I remember a (heated) discussion some time ago about man over board prevention. Well, here is a BBC video of what can happen in a split second if you don't wear you harness and lanyard.

Proof that crossing oceans in large vessels does NOT give any guarantee of safety over doing so in much smaller ones.  ;D

What a poor cockpit design....

  Elevated cockpit, no real combings... lost of downhill slope to head over. 

Not too many small boats designed as poorly as that.
